This is the occupational therapy form YOU need to support guided CONSULTATION / COLLABORATION / DATA COLLECTION with teachers, paraprofessionals and education team! This form is intended to help guide occupational therapists while consulting with teachers for monthly consultation or gathering information needed for an evaluation. During monthly consultation or evaluations, sometimes it is hard to think of questions or discussion topics or teachers may state they have “no concerns”. This form is intended to help you get the information you need to not only document your consultation but guide your consultation questions.

You can give this form to teachers beforehand or you can use this when you are speaking with teachers. This can also be used for evaluations to gather data that you need from teachers.

This resource includes:

•Short form/general form– perfect when you just want to quickly get the data you need. This is one QUICK page for teachers to fill out. There is a second page that you can make notes on. (pages 4-5)

•Preschool consultation form (pages 7-8)

•Elementary consultation form (pages 10-11)

•Middle school consultation form (pages 13-14)

 

NOTE: THIS IS NOT EDITABLE. You will have to print this form to give to teachers or handwrite to fill in yourself. There is no clipart attached to these forms: this is a basic form that is used to give you data you need.

I am a school occupational therapist who has a doctorate in occupational therapy. I work closely with special education and general education staff adapting material. This is where I get my inspiration for this material. I have used my materials with a variety of children with different disabilities including learning disabilities, dyslexia, autism, sensory processing disorder, ADD, ADHD, visual impairments, deaf/hard of hearing, cerebral palsy, down syndrome and more.
