Occupational Therapy Hand Skills Quick Screen / Guided Data Collection School Based OT

Do you ever find yourself evaluating a student and wishing you had a form to document hand skills? HERE IS YOUR FORM! This is 3 pages and includes 9 separate categories of hand skills. This includes questions for skilled observations (ie: When using tool that requires open web space, can they maintain open web space? Can the child isolate their index finger to point or explore items? Can the child isolate their thumb for finger play or thumbs up?) As you can see by these sample questions, this looks at hand skills, over functional activities.

•This is intended as a printable data sheet (3 pages total) for you to use to check off information to help to organize your data when observing or during play based assessments. Questions include checking yes / no / N/A (if not appropriate) and if the items are consistent or inconsistent. If you check “no” and the skill should be developmentally appropriate, this may be an area of further investigation. This DOES NOT include activities to test each skill, nor does it give the developmental level for each item. This form goes over basic hand skill categories including:

3.In hand manipulation

4.Hand dominance / crossing midline

5.Hand arches

6.Radial / ulnar separation

7.Grasping patterns

8.Bilateral skills

9.Individual finger control
