Retained Primitive Reflexes and Handwriting: What OTs, Teachers & Parents Should Know

Handwriting is much more than holding a pencil and forming letters. A child also needs postural stability, bilateral coordination, visual-motor skills, hand strength, motor planning, and the ability to maintain a comfortable position while the eyes, head, hands, and body work together.

When handwriting is difficult, occupational therapists look at the whole child and the demands of the task. Pencil grasp and letter formation are important, but they are only part of the picture.

One area that may be considered as part of a broader occupational therapy assessment is the presence of retained primitive reflex patterns.

Primitive reflexes are automatic movement responses that support early development. As the nervous system matures and children gain increasingly voluntary control of their bodies, these early movement patterns typically become less dominant.

When reflex-related patterns remain influential beyond their expected developmental period, they may be one factor contributing to difficulties with posture, coordination, visual-motor tasks, or efficient movement.

Importantly, handwriting difficulties do not automatically indicate a retained primitive reflex. There are many possible reasons a child may find handwriting challenging, and reflexes should be considered within the context of the child’s overall development and participation.

Why Handwriting Requires the Whole Body

It is easy to think of handwriting as a fine motor skill, but effective handwriting begins with a stable and adaptable foundation.

During writing, a child may need to:

  • Maintain a comfortable seated position
  • Stabilize the trunk and shoulders
  • Keep the head upright while looking at the paper
  • Move the eyes between a model and their own work
  • Use one hand to write while the other stabilizes the paper
  • Reach and work across the midline of the body
  • Adjust pencil pressure and finger movements
  • Coordinate visual information with hand movements
  • Sustain effort long enough to complete the task

When one or more of these underlying skills require significant effort, handwriting may become slower, more tiring, or less efficient.

This is why occupational therapists often look beyond the pencil when trying to understand handwriting challenges.

What Might Retained Primitive Reflexes Have to Do With Handwriting?

Primitive reflex patterns involve automatic relationships between movement, body position, and muscle activity.

During infancy, these patterns serve important developmental purposes. Over time, children typically develop greater ability to move the head, trunk, arms, and legs independently and intentionally.

If an early reflex pattern continues to strongly influence movement, certain positions or tasks may require additional effort.

For handwriting, this may show up as difficulty maintaining posture while moving the arms, coordinating the two sides of the body, crossing midline, or keeping the head and upper extremities working independently.

Rather than asking, “Which reflex is causing poor handwriting?”, a more useful question is:

“What is making this handwriting task difficult for this particular child?”

Reflex patterns may be one piece of that larger clinical picture.

ATNR and Handwriting

The Asymmetrical Tonic Neck Reflex (ATNR) creates an early relationship between head turning and movement of the arms and legs.

During infancy, turning the head toward one side is associated with extension of the arm and leg on the face side and flexion on the opposite side. As voluntary motor control develops, children become increasingly able to turn their heads without their arms automatically following that movement pattern.

For a child with signs of a retained ATNR pattern, tasks that require the head and arms to work independently may be more challenging.

During handwriting, you might notice:

  • Difficulty crossing the body’s midline
  • Frequently switching hands during writing or drawing
  • Turning or rotating the paper significantly
  • Moving the entire body rather than reaching across the page
  • Difficulty stabilizing the paper with the helper hand
  • Challenges copying from a board or another location
  • Reduced efficiency when the head turns while the hand continues working

These observations alone do not confirm a retained ATNR. They can have many possible causes and should be interpreted as part of a broader assessment.

STNR and Handwriting

The Symmetrical Tonic Neck Reflex (STNR) helps support an infant’s transition toward hands-and-knees positioning and crawling. It involves a relationship between head position and movement of the upper and lower body.

Because STNR involves coordination between the head, arms, and legs, retained patterns may be considered when a child has difficulty maintaining an efficient seated position during desk work.

During handwriting, you might notice:

  • Slumping over the desk
  • Frequently changing position
  • Propping the head on one hand
  • Sitting with the legs extended rather than comfortably positioned beneath the desk
  • Moving very close to the paper
  • Difficulty maintaining posture during longer writing assignments
  • Writing performance that decreases as postural fatigue increases

Instead of repeatedly reminding a child to “sit up straight,” it is important to consider why that position may be difficult to maintain.

Sometimes changing the environment, seating, writing surface, movement opportunities, or task demands can make writing significantly more accessible.

TLR and Handwriting

The Tonic Labyrinthine Reflex (TLR) is associated with the body’s response to head position relative to gravity and early flexion and extension patterns.

As children develop, they gain increasingly refined postural control, balance, and the ability to maintain their body position while completing tasks.

When a child demonstrates signs consistent with retained TLR patterns, an OT may also consider areas such as postural stability, balance, body awareness, and head control.

During handwriting, you might notice:

  • Difficulty maintaining an upright seated posture
  • Leaning heavily against the desk or chair
  • Using excessive effort to stabilize the body
  • Fatigue during seated work
  • Difficulty maintaining head position while looking down at the paper
  • Reduced endurance for tabletop activities

For some children, supporting the body first can make hand use easier.

This might include appropriate seating, foot support, movement before writing, alternative writing positions, or opportunities to change position throughout the task.

Palmar Reflex and Pencil Control

The Palmar Grasp Reflex is an early reflex in which pressure or touch to an infant’s palm produces an automatic grasping response.

As hand development progresses, children develop increasingly voluntary control of the fingers and thumb. This allows the hand to adjust force, manipulate objects, and eventually manage tools such as crayons, pencils, scissors, utensils, and fasteners.

When an OT suspects that retained reflex patterns may be influencing hand function, the Palmar Reflex may be one area considered alongside strength, dexterity, sensation, joint stability, motor planning, and experience with fine motor tasks.

During handwriting, you might notice:

  • Excessive pencil pressure
  • A very tight or effortful grasp
  • Difficulty adjusting the pencil within the hand
  • Limited separation of the two sides of the hand
  • Hand fatigue
  • Reduced finger isolation
  • Using larger wrist or arm movements instead of refined finger movements

Pencil grasp should also be considered functionally. A grasp does not need to look textbook-perfect to be effective.

The more important questions are whether the grasp is comfortable, efficient, sustainable, and functional for the child.

Moro Reflex, Regulation, and Handwriting

The Moro Reflex is an early protective startle response. Although it is not a handwriting reflex, regulation and arousal can have a significant impact on a child’s ability to participate in writing.

A busy classroom may include chairs scraping, announcements, unexpected touch, people moving nearby, changes in routine, and sudden noises.

For a child who is highly responsive to unexpected sensory input, a writing task may become much harder when the nervous system is also managing everything happening in the environment.

You might notice:

  • Becoming easily distracted or startled during writing
  • Difficulty returning to work after an interruption
  • Increased tension during challenging assignments
  • Avoiding writing in busy environments
  • Rushing through work
  • Fatigue after periods of sustained attention
  • Needing additional time to settle after unexpected sensory events

These behaviors should not automatically be attributed to the Moro Reflex. Sensory preferences, anxiety, attention, learning demands, environmental factors, and many other considerations may contribute.

Supporting regulation and felt safety can be just as important as practicing the motor components of handwriting.

What About Other Primitive Reflexes?

ATNR, STNR, TLR, Palmar, and Moro are some of the reflexes that may come up most frequently when discussing handwriting, but handwriting never occurs in isolation from the rest of the body.

Balance, lower-body stability, sensory processing, visual-motor coordination, body awareness, and overall motor development can all influence participation at a desk.

Depending on the child’s individual presentation, an occupational therapist may consider additional developmental movement patterns as part of a comprehensive assessment.

The goal is not to find as many retained reflexes as possible.

The goal is to understand what is helping or interfering with the child’s participation and determine what supports will make meaningful activities easier.

Signs That a Child May Need More Handwriting Support

A child does not need to have a retained reflex to benefit from occupational therapy support.

Additional assessment may be helpful when handwriting difficulties consistently interfere with school participation or cause significant frustration or fatigue.

Some examples include:

  • Hand pain or significant fatigue during writing
  • Writing that is substantially slower than classroom demands
  • Difficulty copying information
  • Persistent challenges with letter formation or spacing
  • Difficulty coordinating the helper hand
  • Significant postural fatigue during tabletop work
  • Avoidance of written assignments
  • Difficulty keeping up despite appropriate instruction and practice
  • Fine motor or visual-motor difficulties across multiple activities

An OT can help determine whether the primary challenge involves motor skills, visual-motor integration, sensory processing, postural control, environmental demands, task expectations, reflex-related movement patterns, or a combination of factors.


Supporting Handwriting Without Making Every Activity About Handwriting

Sometimes the best preparation for pencil-and-paper work does not involve a pencil at all.

Children can develop underlying motor skills through meaningful movement and play.

Depending on the child’s needs and preferences, this might include:

  • Crawling and obstacle courses
  • Climbing and playground activities
  • Building with construction toys
  • Drawing on vertical surfaces
  • Animal walks
  • Carrying, pushing, and pulling during everyday activities
  • Ball games
  • Crafts and manipulatives
  • Bilateral coordination activities
  • Movement games that involve reaching and changing positions

These experiences provide opportunities to practice coordination, postural control, hand use, visual-motor skills, and body awareness without turning every moment into an exercise.

For children who are being specifically supported for retained primitive reflex patterns, activities can also be intentionally selected to provide opportunities for the movement patterns being addressed.

A Play-Based Approach to Reflex Integration

Reflex integration activities do not need to look like repetitive drills.

Movement can happen through obstacle courses, pretend play, games, sensory experiences, classroom movement breaks, playground activities, and familiar home routines.

Whenever possible, activities should be:

  • Developmentally appropriate
  • Playful and meaningful
  • Responsive to the child’s cues
  • Adaptable to different abilities
  • Free from forced participation
  • Connected to functional goals

A child should not be required to repeatedly perform an uncomfortable movement simply because a reflex is suspected.

The ultimate goal is participation, not simply completing an exercise.

The Bottom Line

Handwriting difficulties are rarely about just one thing.

A child’s pencil grasp, posture, visual-motor skills, bilateral coordination, sensory processing, motor planning, learning demands, environment, and developmental movement patterns may all contribute to how easy or difficult writing feels.

Retained primitive reflexes may be one consideration within that larger picture, but they should never replace a comprehensive look at the child’s strengths, needs, environment, and functional participation.

For occupational therapists, teachers, and families, the most useful question is not simply:

“Does this child have a retained reflex?”

It is:

“What does this child need to participate in handwriting more comfortably, confidently, and successfully?”
